Kankakee County: During these unprecedented and unusual times please remember that Catholic Charities’ Aging & Disability Services Division is still here to support older adults aged sixty and up and those who care for them.
Feeling Isolated?
If you are feeling lonely, isolated, or secluded we can help. Please reach out and we can connect you with our Telephone Reassurance Program. The Program consists of a phone call from one of our team members just to check in, provide some uplifting conversation, and perhaps provide information on supports that may be available to you.
Do You Have What You Need?
Our goal is to help you remain independent and safe in your home environment so let us help with contactless grocery delivery; contactless delivery of medical supplies, toiletries, prescriptions, incontinence supplies, nutritional supplements, or cleaning supplies; or even assistance with transportation to crucial appointments. These supports are at no cost to you and are intended to keep older adults safe and socially distanced to reduce exposure during this unusual time.
Do You Need Support Services In Your Home? Assistance Applying for Benefits?
Contact our office today to be connected with a Certified Care Coordinator or our Senior Health Information Program Team who can assist with an assessment and referral to those supports you need to remain safe and independent.
